Requesting Medical Records

Requesting medical records can be completed in two different ways: online and in person. 

Online: Conveniently request your records online via a secure portal by  clicking the "Request Medical Records Online" button above.

In Person:

• Anyone under 18 years of age must be accompanied by a parent/guardian
• Fill out out Release of Information Form in its entirety
• Must have ID when requesting records

The completed form must be presented to the Health Information Management Department, located on the ground floor of the hospital main building. Regular business hours are Monday through Friday 8:30 a.m. until 4:30 p.m.
