REQUESTING MEDICAL RECORDS
- Anyone under 18 years of age must be accompanied by a parent/guardian
- Fill out out Release of Information Form in its entirety
- Must have ID when requesting records
The completed form must be presented to the Health Information Management Department, located on the ground floor of the hospital main building. Regular business hours are Monday through Friday 8:30 a.m. until 4:30 p.m.
