Registration

When you arrive, please go to the Patient Registration area on the 1st Floor adjacent to the atrium lobby. Upson uses a pager system (similar to the system used by many fine restaurants) to help protect your privacy and expedite the registration process. Please stay within the hospital until your pager alerts you to return to the Registration receptionist. You will then meet privately with one of our registrars. After registering, we will help you get to the right department or unit.

 Please note: Emergency patients who are being admitted will be registered through the Emergency Department.

 Please bring with you:

  • Insurance card
  • Driver’s license or other valid ID
  • Doctor’s orders
  • The name of your referring physician
  • A copy of your Advanced Directives or Living Will
  • Prior approval or pre-certification information (if applicable under your insurance plan)
  • Credit card or check book (for your co-pay)

If you don’t have insurance or don’t have adequate insurance, please let us know. Our financial counselors can help you apply for assistance or other financial relief, and/or set up a payment schedule that meets your needs. 

Inpatient

Upson’s admitting office, located in the front lobby, just off Cherokee Road, is the first stop you will make if you are admitted to the hospital overnight. Even patients who have phoned in advance and pre-registered will need to check in at Registration

Be sure to bring your social security card, identification (driver’s license or photo ID), insurance card, any paperwork you have received from your doctor and your Advance Directives (living will, power of attorney or healthcare surrogate documentation).

Please keep in mind that every time you are admitted or registered to the hospital, our staff will confirm your address and make copies of your identification and insurance cards.

You will want to leave all of your valuables – wallet, money, credit cards, jewelry and similar items at home or give them to your family members at the time of admission.

An Upson financial counselor is available should you have questions about insurance or billing. Co-pays or deductibles are requested at the time you receive hospital services.

Outpatient

If your doctor has ordered an outpatient test or procedure (examples: mammogram, blood test, chest X-ray), please pre-register by calling Upson’s Central Scheduling at (706) 647-8111 extension 1114. You will be given an appointment time and date, and any necessary instructions.

The day of your procedure or test, be sure to bring in your social security card, proof of identification (driver's license or photo ID), insurance cards and the outpatient prescription you received from your doctor.

Co-pays or deductibles are due at the time you receive hospital services.
